Our office is located at 1140 E. 3900 S., Suite 340, on the 3rd floor of the Women's Pavillion at St. Mark's Hospital, Salt Lake City, UT

When You Arrive

Park in Visitor or Patient parking by the Women's Pavillion and come to main entrance. Take the elevators to the 3rd Floor, Ste. 340.

Driving Directions

From the North:
Take I-15 South to the I-80 interchange. Exit I15- to I-80 East at 700 East and turn right, heading south. Follow 700 East to 3900 So. Turn left on 3900 South and head east. You will see the hospital on your right, about five blocks down.

From the South:
Take I-15 North to the 4500 South exit. Exit I-15 at 4500 South and turn right, heading east (toward the mountains). Follow 4500 South to 700 East. Turn left on 700 East, heading north. Follow 700 East to 3900 So. Turn right on 3900 South, heading East. You will see the hospital on your right, about five blocks down.

From the East:
Take I-80 West to 700 South, Exit I-80 at 700 East and turn left, toward the south. Follow 700 East 3900 South. Turn left on 3900 South, Heading east (toward the mountains). You will see the hospital on your right, about five blocks down.

From the West:
Take the I-80 East to I-15 South. Take I-15 South to the I-80 East interchange. Follow I-80 East to the 700 East exit I-80 at 700 east and turn right, heading south on 700 East. Follow 700 East to 3900 South. Turn left heading east (toward the mountains). You will see the hospital on your right, about five blocks down.
