After Hours Emergencies
If you are experiencing a medical emergency, please Dial 9-1-1 . However, if you need after hours advice for a medical concern, please call (801) 743-4730 and our answering service will assist you.
Prescription Refills
We prefer to refill your medications at the time of your office visit. Please bring your medication list with you to each appointment.
Forms Completion
We require a 48 hour notice on all forms, such as work release or Family Medical Leave Act paperwork, to be filled out. We will notify you once the form has been completed.
Your Medical Records
Your medical records are confidention and will not be released without a signed authorization from you. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
