Clinic days may fluctuate depending on the physician's surgical schedule.
Appointments
To schedule an appointment, please call us at (801) 743-4730 during regular office hours. Please call in advance for non-urgent appointments so that we may schedule a date and time to fit your convenience. We recommend you schedule your next follow-up appointment at the conclusion of your office visit.
Please bring your health insurance card to your initial visit, as well as subsequent visits. In addition, please bring:
- a list of medications you are taking
- a list of known drug allergies
- relevant immunization or medical records
Your first appointment will include an extensive history and physical exam, review of any scans and/or x-rays ordered by your referring doctor, laboratory review and a detailed explanation of interventions recommended. You will need to provide accessibility to previous scans ordered pertinent to your diagnosis, all biopsy results pertaining to your visit, and any additional information requested upon scheduling the appointment.
If you are a new patient, please arrive 30 minutes before your scheduled appointment time to complete your paperwork. You may also download our patient registration forms in advance, fill them out, and bring them with you when you come for your appointment.
Cancellations
We call to confirm your appointment one business day prior to your planned visit. In the event that you must reschedule an appointment, please notify our office at least 24 hours in advance to avoid a cancellation fee.
If you are going to more than 15 minutes late it may be necessary to reschedule your appointment.
